In compliance with the latest SEN Code of Practice (2015), our school SEN register is fluid and designed to provide children with relevant support.
We use a triangulation process to identify children on the SEN register:
- We trace all children on Target Tracker and through termly pupil progress meetings to identify children not making sufficient progress.
- The SENCO provides teachers with the TKAT "Evidence for requesting placement of a pupil at SEN support" grids and, following evidence of class action, identifies pupils making inadequate progress or having difficulties in three of the key age related areas within: Cognition and Learning, Social Emotional and Mental Health, Communication and Interaction, or Sensory and Physical.
- The SEN team maintain comprehensive records of all external professional involvement with our children, we audit these regularly to ensure that all recommendations from professionals are fulfilled.
